Ōtaki Medical Centre, November 2015

F_R_OtakiMedicalCentreDr Hans Itjeshorst has resigned from our medical centre and his last working day with us is Thursday October 22nd. We are actively looking for another GP to join our practice but this could take some time so until then we will be closing our books to new patients.

Dr Itjeshorst’s existing patients will continue to be looked after by our Medical Team but as we do not currently have a replacement for Dr Itjeshorst they will be given the choice of those GPs who are available at the time they require a consultation.

Closing our books

Closing our books was not a situation we ever wanted to find ourselves in but first and foremost we must provide care for our existing enrolled patients. If you are not enrolled with us but were planning to enrol we will be able to see you as a casual patient until such time as our books are re-opened.

Unfortunately we do not receive funding from the Ministry of Health for non-enrolled patients so we are unable to provide subsidised health care for casual patients.

Consultation fees

The GP consultation rates for casual patients are:

  • Adult Consultation $65 & $50
  • Child 13yrs and over $50 & $45
  • Child under 13 (incl ACC) free & free
  • ACC — 13 yrs and over $35 & $35

Call out Fees (excluding under 13’s) (in addition to the consultation fee)

  • Home Visits $20 (during surgery opening hours)
  • After Hours $65 (during non-surgery hours)
  • Saturday clinic surcharge $10.00 (excluding under 13’s)

Nurse charges

  • Children under 13 (enrolled & casual, incl ACC) — Free
  • Casuals, 13yrs and over (includes ACC) — $35

Letter from Dr Itjeshorst

Dear patients,
After working as a general practitioner at Ōtaki Medical Centre for the past seven-and-a-half years, I have decided that it is time for a change. My wife and I are looking at doing some voluntary work overseas and pursue other opportunities. This means that I will stop working as a GP at Ōtaki Medical Centre and my last day in the practice will be 22 October.

I have had a great time at OMC and met a lot of nice and interesting people. Ōtaki is a great place with wonderful people. I am sure the practice will find a good successor to help you manage your health and with the wonderful staff OMC you will be in good hands.

I thank you for your trust in the past seven-and-a-half years and I wish you all the best for the future.

Kind regards,
Hans Itjeshorst
